Pay your attention:

1.  A certificate of temporary disability in case of illness is issued upon personal visit to the therapist of a medical health center.

2. To extend or finalize the certificate of temporary disability, you must personally visit the therapist at the medical health center on the appointed day.  

If a student, who has been issued a certificate of temporary disability (at the medical center) fails to appear for an appointment with the therapist on the appointed day, a note will be made in the electronic journal “violation of the regime:”, type of violation (“failure to appear for an appointment”). In this case, the exemption from classes ceases to be documented and is considered an absence without a valid reason.

3. If you start classes before the permitted period of disability, the certificate of temporary disability must be finalized upon personal visit to a therapist, so that the period of your absence is considered officially permitted.

4. A certificate of temporary disability cannot be issued retroactively.

5. Students, who become temporarily disabled during the examination session, must provide the dean's office with a medical certificate of health on paper.

To register certificates of temporarily disability from other medical institutions (polyclinics/hospitals), you must personally contact the medical center at 11, BLK str. (hostel №5) or send it to the phone number via Viber or Telegram +375-33-99-00-848

Pay your attention that medical certificates issued by other medical centers will be registered from 14.00 до 16.00.

We remind you that in order to be exempt from PE lessons, the medical certificate of health or the certificate of temporary disability must indicate the period of restriction from physical activity. If the period is not indicated, the exemption from PE lessons will not be registered.

To get the extract from medical documents (form №1 мед/у-10, approved by the Resolution of the Ministry of Health of the Republic of Belarus № 92 of 09.07.2010 “On the establishment of the forms “Medical certificate of health condition”, “Extract from medical documents” and approval of the Instructions on the procedure for filling them out”), please contact Hostel №5 (11, BLK str.).

The extract is issued within 5 working days since the date of application.

The basis for issuing the extract from medical documents is clause 7.9. of the List of administrative procedures carried out by government agencies and other organizations upon application of citizens, approved by Decree of the President of the Republic of Belarus № 200 of 26.04.2010.

Dear students, we remind you that:

1. When a student contacts a medical center regarding illness, with subsequent issuance of a certificate of temporary disability, the student must come to a follow-up appointment with the therapist of the medical center on the appointed day, specified by the doctor of the medical center to extend the certificate or finalize it.

A student's failure to appear for a follow-up appointment on the day assigned to him/her is a violation of the outpatient regime, so the certificate is considered valid only on the day of the initial visit to the health center (first visit). All other days of missed classes are considered missed without a valid reason.

2.  During the session, in order to postpone an exam due to illness, the student must provide the dean's office with a certificate of temporary disability of the established form from a healthcare organization (polyclinic or medical center) on paper.
